4. Event Overview

   Help make this year’s “Halloween at the ‘Fest” event, on October 19th, a smashing success by being a Trick-or-Treat participant!

   This year we’re turning the forest at Riverfest Park into a Trick-or-Treat Trail. You will decorate a table or area along the trail and provide a spooky atmosphere for children to enjoy as they collect candy, goodies, or your organization’s promo materials. This is also a great opportunity for your organization to advertise that you’re looking for volunteers or new members!

   We will have a "Hallow-scene Supreme" (best dressed) contest for all participating stations!

   Registration is free, however all registrations are due by 5pm on October 14th.

18. Please confirm that you adhere to the following policies:

    1. Participants must arrive no later than 4:15pm and have stations decorated and ready to go by 4:45pm.
    2. Treats are supplied by the participant
    3. Only pre-packaged candy or treats are allowed
    4. No scary displays
    5. All stations must be registered with the City of De Soto
    6. An adult must be with their station at all times during the Trick-or-Treat for safety reasons
20. Waiver of Liability

    The undersigned hereby agrees to defend, indemnify, and hold harmless the City of De Soto and its employees and agents from and against any and all loss, liability charges and expenses (including attorney fees) and cost which may arise by reason of participation in any City program. (The City does not provide accident, medical, liability, workers’ compensation insurance or any other insurance for program participants). As parent/guardian, I hereby consent to emergency treatment of my minor child as a result of accident or injury. I further agree to pay any and all cost incurred as a result of said treatment. I agree to carefully inspect and satisfy for myself that the facilities provided are reasonably safe for their intended use. Once having conducted the inspection, I agree to expressly assume the risk of participating at the premises. I understand the City retains the right to use photos taken during activities for publicity purposes.

    I acknowledge that my participation in this event, or that of my child, could involve risk of physical injury or damage to property. I expressly assume such risk and release and waive any claims against the City of De Soto, its agents and employees for any injuries to persons or damage to property.

22. Electronic Signature Agreement

    By checking the "I agree" box below, you agree and acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, and 3) you may still be required to provide a traditional signature at a later date.
